What should a fault report form include?

A fault report form should record who is reporting and how to reach them, the site, room and exact device with its serial or asset code, the symptom in plain words, when it started and whether it is intermittent, the impact on users, what has already been tried, and a photo or video, followed by the installer's priority and triage notes.

How do you get the most from a fault report form?

Put a QR code on the kit

A label on each device that opens the fault form means the report arrives with the right serial and room, instead of "the charger by the back door".

Ask what changed

Building works, a power cut or someone else's network change can all look like a fault in your kit. Asking up front can save a visit, or show the fault isn't yours.

Keep the customer's part short

Customers won't fill in 20 questions. Keep their sections to the essentials and leave priority, cover and triage to your office.

Check cover before booking

Decide whether the fault is under warranty, a service agreement or chargeable at triage, and tell the customer before the engineer travels.

Is this fault report form free?

Who should fill in a fault report form?

Whoever finds the fault. The first four sections are written for a customer or end user; the triage section is for the installer's office. Engineers can use the same form when they find a fault during another visit.

What's the difference between a fault report and a service visit report?

The fault report comes first: it describes the problem so it can be prioritised. The service visit report comes after: it records what the engineer checked, found and fixed on site.

Do we have to fix faults after handover?

Your contract and any warranty set most of it, so check them. In England, Wales and Northern Ireland, a business supplying a service has an implied duty to carry it out with reasonable care and skill, and the Consumer Rights Act 2015 treats every consumer service contract as including the same term.
